Your Health Information Rights 

You have the following health information rights: 

  • Request restrictions on certain uses as well as disclosures

  • Choice and Law; Venue: Parties submit to Texas jurisdiction and agree to bring legal actions related to this agreement in Texas. 

  • Severability: If any part of this agreement is unenforceable, the rest remains in effect. 

  • Electronic Signatures: You agree electronically when logging in.  

  • Counterparts: It is possible to execute this agreement in more than one counterpart.

  • Nature of the Parties' Relationship: Parties are independent contractors. 

  • Interpretation: Ambiguities are resolved to permit compliance with Healthcare Insurance Portability and Accountability Act (HIPAA) Rules.
